Tina -- I used to think I was awesome for suggesting an integrated wellness system including the management of chronic conditions (e.g., this blog entry from a few years ago: http://urbanplacesandspaces.blogspot.com/2008/02/dispuptive-innovation-once-again.html) but Christopher Alexander posited something not dissimilar in _Pattern Language_ which was published in 1979. (FWIW, a couple decades ago I worked for the Center for Science in the Public Interest, where I learned about health and wellness, focused on nutrition and alcohol policy primarily.)
